Suggestions for Use and Completion of Mid-Year Goal Review/Revision and Ongoing Performance Feedback Forms

Mid-Year Goal Review/Revision

This form is used to facilitate communication between the supervisor and employees in the review/revision of performance goals. This process occurs at mid-year or during the sixth and seventh month of the performance appraisal year.

This process provides the opportunity to:

  1. Review progress made towards established goals.
  2. Delete or revise goals based upon program priorities, time/resource availability, etc.
  3. Establish new goals if the original goals have been met.

The Ongoing Performance Feedback Form may be used to capture and track significant performance feedback points discussed with employees during regular update meetings.
