This is the section in which you describe the employees overall job-related performance, including safety-related activities. You may choose to list the employees job functions, addressing each of them separately, or you may write the appraisal in a narrative form.
Special appraisal categories remind us that some positions have additional responsibilities associated with them and that these responsibilities should be addressed in the appraisal process.
Example: If you are completing an appraisal report for supervisor, remember to address the "supervisory skills" of that person. Place an "X" in the box next to "Supervisory Skills" and either incorporate your comments into the text of the narrative or attach a separate sheet with your comments.
The summary of accomplishments is not a self-appraisal. It is a tool the employee uses to list or describe actions taken or work performed that has led to the achievement of stated goals or other significant contributions to the work and mission of the unit. This summary is intended to assist the supervisor in addressing and documenting accomplishments, which also should be mentioned in appraisal comments.
Employees compiling their summaries of accomplishments will be instructed by their supervisors to:
- Identify goals achieved and performance expectations met.
- Note training and development opportunities and their impact.
- Note significant customer feedback recognition.
- Note ideas/suggestions given or actions taken that resulted in process improvements or measurable efficiencies.
- Note activities related to equal opportunity, affirmative action and diversity that contribute toward building community.
Employees goals are developed in partnership between the supervisor and employee. Goals should align with the mission and goals of the department and with "Balanced Scorecard."
Employee goals should be measurable and attainable.
On Supervisor Support section of the goal sheet, document specific action steps or areas of support to be provided by the supervisor that will enable achievement of the employees goals.
